Title: Lean on Me: A Case-Based Approach to Multi-Disciplinary Care in Cancer Survivorship
Course Description: There are an estimated 20 million cancer survivors currently living in the United States. Increases in cancer screenings, advanced surgical technology and improved targeted therapies will continue to advance the growing number of people who live beyond active treatment of cancer. Cancer treatments result in a plethora of both short term and late sides effects that can interfere with a person’s return to prior function and negatively impact their quality of life. Physical therapists are uniquely qualified to address the concerns of cancer survivors, including cancer related fatigue and fear of cancer recurrence. However, in order to effectively address these late effects requires a collaborative approach that includes dietitians, social workers and advanced practice practitioners. This Continuing Education session will emphasize the importance of a multidisciplinary approach to cancer survivorship and educate the learner that in order to provide holistic, comprehensive care for our patients, we need to collaborate with other health care providers.
Learning Objectives:
- Identify the common themes in long term cancer survivorship
- Describe the roles of the physical therapist, dietitian, and social worker in addressing the long-term needs of cancer survivors
- Postulate on the learner’s ability to engage and collaborate with other allied health professionals in meeting the long-term needs of cancer survivors
Kristen Madrid, LCSW, OSW-C
Kristen is a board-certified clinical oncology social worker with several years of experience specializing in breast oncology and survivorship care at the Yale Cancer Center Survivorship Clinic and Smilow Cancer Hospital. Her work has focused on supporting individuals and families as they navigate the emotional, psychological, and practical challenges that accompany a cancer diagnosis and life beyond treatment.
Kristen is the owner of Resilience Through Cancer LLC, a behavioral health practice dedicated to providing compassionate, evidence-based care for individuals impacted by cancer while aiding in addressing the complexities of survivorship.
Natalie Smith, MS, RDN, CSO,
Natalie Smith is a Registered Dietitian who is also a Certified Specialist in Oncology. Natalie graduated from UCONN with her Masters in Health Promotion and Education and became a Registered Dietitian Nutritionist in 2014. Immediately after graduation, Natalie began working as an Outpatient Oncology Dietitian and now has 12 years of experience in Oncology Nutrition. She sat for her Certified Specialist in Oncology exam for the first time in 2021 and recently passed it again in 2026. In 2022, Natalie also opened her own nutrition private practice called Nutrition by Natalie, LLC and counsels patients 1:1 focusing on diet and lifestyle changes for weight loss, high cholesterol, diabetes, oncology survivorship, and many other conditions and illnesses.
Scott Capozza, PT, MS
Scott Capozza, PT, MS works as a physical therapist dedicated to oncology rehabilitation at Smilow Cancer Hospital at the Yale Cancer Center in Connecticut. He sees patients in the outpatient setting as well as in a multi-disciplinary long term survivorship clinic.
Scott is among the first class of board-certified clinical specialists in oncologic physical therapy in the United States, and currently serves on the ABPTS Oncology Standard Setting committee. He holds several leadership positions within the American Physical Therapy Association, including the APTA Media Corps and the Academy of Oncologic Physical Therapy.
Scott has taught as an adjunct professor in Oncology Physical Therapy for both Quinnipiac and Sacred Heart Universities in Connecticut and has delivered lectures to physical therapy programs around the country about the benefits of oncology rehabilitation. He has served as a CI in both pediatrics and oncology and is an APTA Level I Credentialed Clinical Instructor. He has presented professionally on the local, state, national and international level and has been a co-author on several peer-reviewed publications. He is also the co-author of the chapter ‘Young Adult Survivors’ in the textbook Oncology Rehabilitation: A Comprehensive Guidebook for Clinicians.
